All SRTC training scholarship applications
must include the following items:

1. Completed application form. An electronic
version of this form must be submitted
by email as well. Click

form in Microsoft Word format.
2. A curriculum vitae. Must submit in common CV- CIHR format with attachment. Draft format is not accepted.
3. Official or certified copy of University transcripts.
4. For proposed research supervisors
who are not currently within the CIHR-SRTC,
please also submit a CIHR-formatted CV
for the proposed supervisor (see www.commoncv.net).
Completed applications should be sent
